How do I register to baptize my child at St. Matthew?

Please submit the following:

1. Registration Form (English, Spanish)
2. Copy of child's Birth Certificate (not the original)
3. Signed Godparent Declaration of Eligibility (English, Spanish) with witness' signature. The declaration must be completed in the presence of a priest, religious, or lay parish staff member. A witness signature is required.

Where do I submit my documents?
Documents may be dropped off at the parish office or submitted via email to [email protected]

Registration is due the Friday 1 month before the desired date of Baptism. Registration may be submitted earlier. Any registration submitted later than 1 month before the desired date of Baptism will be processed with the next month's registrations. We will be unable to honor the desired date of baptism.

 What preparation is required for baptism?

Parents and godparents need to prepare for the Sacrament of Baptism.
Preparation is one 2-hour meeting.
English--generally every 4th Thursday of the month starting July 2024
Spanish--generally every 4th Monday of the month

Upcoming Preparation Meeting Dates:

Starting July 2024, English meetings will take place from 7:30-9:30 pm in Parish Hall Room D.
Spanish Meetings are from 7-9 pm in Parish Hall Main Room.

Upcoming Baptism Dates:
Baptisms are on Saturday at 11 am.

We aim to honor your desired baptism date from the available dates listed above. However, we will be unable to do so if registration is submitted less than one month before the desired date. 

 If your registration is incomplete (form filled out incomplete, missing birth certificate, missing or incorrectly completed Godparent Declaration of Eligibility), we will be unable to honor your desired date of baptism. Please review all required documents carefully before submission. If you need assistance with your registration documents, please get in touch with our office.

Frequently Asked Questions
If I have previously been a godparent or had another child baptized, do I still need to participate in preparation for the Sacrament?
 
If you have previously prepared for the Sacrament of Baptism at St. Matthew as a parent or godparent, please inform us what year you participated. If it has been less than two years since you prepared, you do not need to go through preparation again, though you are welcome to participate if you would like.
 
If you have previously prepared for the Sacrament of Baptism at another parish as a parent or godparent, please provide proof of participation (typically a letter or certificate from the parish). If you are unable to obtain proof of participation from another parish, we ask that you join us in the preparation process for a review in understanding the Sacrament of Baptism.
 
I want to baptize my child at another parish, but I need to prepare for the Sacrament; what steps do I need to take?
 
Please submit a registration form and write "at another parish" on the line Desired date of baptism. After completing preparation, you will receive a letter of preparation to provide the parish at which you have the baptism.
 
I have already prepared for Baptism at another parish and need to schedule it. What paperwork do I need to turn in?
 
If you have already prepared for the Sacrament of Baptism, please submit the following paperwork to schedule a Baptism at St Matthew.

2. Copy of Child's Birth Certificate
3. Godparent Declaration of Eligibility (link at top of page)
4. Proof of Preparation from another parish (typically a letter or certificate)

 
I am visiting from another state to baptize my child, what steps do I need to take?
 
In addition to the paperwork listed in the previous question (I have already prepared for Baptism...), please submit a letter of permission from your pastor granting permission to have your child baptized in our Archdiocese.
